THE PROMISE

No more bags. A smoother under-eye. A rested, refreshed look.

What is Lower Blepharoplasty

With age, the fat pads that cushion the eye can bulge forward to form under-eye bags, while the skin loses elasticity and the boundary between the lid and cheek hollows — casting shadows and dark circles that make you look tired even when you are not. Lower blepharoplasty, or lower eyelid surgery, addresses these changes to smooth the under-eye and restore a rested appearance.

Depending on your anatomy, the procedure is performed through an incision just below the lower lash line, or from inside the eyelid — the transconjunctival approach, which leaves no visible scar. Through it, the fat pads are repositioned or carefully reduced and any loose skin is tightened. It is carried out under sedation, and usually takes around 45 minutes.

The Procedure

Rather than simply removing fat, modern lower blepharoplasty often repositions the fat pads to fill the hollow between the lid and cheek, which avoids a hollowed, over-operated look and gives a smoother, more natural transition. Where appropriate, a small amount of fat is removed and loose skin is tightened. The procedure is frequently combined with a mid-face lift, when the cheek has descended, and with fat transfer or SVF to restore volume and improve skin quality around the eye.

How the Patient Recovers

Most patients are ready to return to their everyday activities within about one week. Some swelling and bruising around the eyes is normal in the first days and settles gradually; sensitivity to light and watering of the eyes can be expected early on. Strenuous activity and alcohol are best avoided for the first few weeks, and contact lenses can usually be resumed after about two weeks, upon reevaluation by the doctor. We use the most advanced surgical technology, disposables and techniques to maximise your results and shorten your recovery. To reduce risks and protect your result, we recommend refraining from smoking for a minimum of six weeks before surgery and throughout your recovery period.

Are You the Right Candidate?

Lower blepharoplasty is well suited to individuals with under-eye bags, puffiness, or hollow, dark circles that persist regardless of rest. When the cheek has also descended, it is often combined with a mid-face lift for a smoother lid-to-cheek transition.

To determine if this procedure is right for you, please schedule a consultation with Dr. Karatzias. During the consultation, your concerns will be discussed in depth and the best surgical or non-surgical solution will be determined, always having your trepidations and expectations in mind.

BEFORE & AFTER CASES

Frequently Asked Questions

Will I still look like myself, or will it look “done”?

The aim is a rested, refreshed version of your own eyes — not a hollow or operated look. That is why the fat is usually repositioned to fill the lid-to-cheek hollow rather than simply removed, which keeps the transition smooth and natural. Done well, people tend to say you look less tired without being able to say why.

Will I have a visible scar?

Often not. When the work is done from inside the eyelid (the transconjunctival approach) there is no external scar at all. If an incision just below the lash line is needed instead, it sits in a natural crease and typically fades to be very hard to see. The right approach for your anatomy is chosen and explained at your consultation.

Does it hurt, and what will the bruising and swelling be like?

Most people describe tightness and mild soreness rather than real pain, settled with simple pain relief. Some swelling and bruising around the eyes is normal in the first days and eases gradually, and you may notice light sensitivity or watering early on. A few people bruise more than others, so it is wise to keep the first week or two free of important events.

How much time should I take off work?

Most patients feel ready for everyday activities within about one week, by which point the bulk of bruising has usually settled enough to cover or conceal. Strenuous exercise and alcohol are best avoided for the first few weeks, and contact lenses can usually be resumed after about two weeks once the doctor reviews you.

How long do the results last?

They are long-lasting. Removing or repositioning the under-eye fat pads addresses the bags durably, so the puffiness does not simply come back. Your face still ages naturally over the years, but most people are happy with a single procedure and do not need it repeated.

Can it be combined with other procedures?

Yes, and it often is. When the cheek has also descended it is commonly paired with a mid-face lift for a smoother lid-to-cheek transition, and fat transfer or SVF can be added to restore volume and improve skin quality around the eye. Combining where it makes sense means one recovery instead of several — what suits you is discussed at your consultation.
